Dear Ms. Maksimowicz,
Perhaps I need to remind you that Sedgwick took over
administration of Xerox Disability on October 1, 2006. They have
nothing to do with Xerox cutting off my disability benefits on April 1,
2006, and failing to restart them or pay my back benefits when SHPS
reauthorized them. I have attached a copy of the letter from SHPS.
I have talked to a number of people at Sedgwick and
Xerox Benefits and they all tell me that I need to go through
you. In fact, my email asking for someone else to contact,
received the following reply:

Subject: Long Term Disability
My LTD payments were discontinued 4/1/06. I won my appeal
in October 2006. I have still not received any continuing or back
payments. Sedgewick/VPA/XIAP are not returning my calls. The Xerox
contact I was given, Linda Max[something] is not returning my calls.
Can you give me some contact who can find out what the
problem is?
As I pointed out last letter, you claimed that
Patrick Harrison has been trying to contact me, while he denies
it. He claimed to be unfamiliar with my case on February 28th
when I called him shortly after I talked to you. He said that
members of “his staff” had called me. He said he would look into
the status of my case and call me back that afternoon. He did not
call me back, that afternoon or since then. I called him on March
2nd as well as calling you, and left you both messages which neither of
you returned.
On March 6th, Tracy called me, saying she’d gotten a
message that I had called her, which I had not. She told me that
she knew nothing on my case and would have to talk to Patrick, and she
would call me back that day. She did not call back and has not
called back since. She said the same thing on February 5th also
promising to look into and call me back and not doing so.
Am I expected to believe that she found not opportunity to talk to
Patrick during that month’s time, in spite of my repeated phone calls
asking for information? Even when my phone calls began in
December?
The one thing that did happen as the result of my
reaching Patrick on the 28th was that he sent Sedgwick paperwork for me
and my doctor. The one thing that Tracy was able to tell me on
the 6th is that the presence or lack of this paperwork has nothing to
do with the resolution of my case. However, the paperwork has
been faxed and received by them, and I am including a copy of my
doctor’s report here for your records.
